By sending the form, you consent to the above-mentioned responsible body collecting and processing your personal data electronically.

Pursuant to Art. 6 section 1 sentence 1 lit. c GDPR, the collection and processing is carried out by the competent authorities to fulfil legal obligations to enable contact tracing if necessary. Any use and disclosure for other purposes is prohibited.

Your data will be automatically deleted after expiry of the legal retention period. The retention period is: 4 weeks

You have a right of access, rectification, cancellation, restriction of processing, opposition to the processing of your personal data, as well as the right to data transfer (Art. 15 to 21 GDPR). For the collection and storage of data, the responsible body uses the service provider Webmasterware.net GmbH & Co KG, Gartenfeldstr. 6-10, 54295 Trier ("WMW").
